1. Complete order form.
2. Return the form to school in a SEALED envelope. Written on the envelope should be:
"Hershey Park form"
Student's name
Teacher's name
Parent email
Parent cell phone
3. Inside the envelope should have your completed form, cash/check or write "PayPal" along with the email or phone number associated with the payment.
Send in cash, check or pay electronically.
